New Patient Registration
With effect from 1st April 2004 all patients within our Practice boundary are registered with the Practice rather than with a particular doctor, but a particular doctor may prefer to look after the same individual throughout a particular course of treatment. You are able to express a preference of GP and whilst the Practice will try to comply, it might not always be possible, in which case we will explain why.
When you do register we ask you to fill in a questionnaire that will give us the details of your medical history – please complete it as fully as possible.
How to Register
All registrations must be done in person at the surgery during the times detailed below. To register at the Practice you need to live within the Practice boundary (High Wycombe area), for everyone over the age of 16. You must be able to prove who you are and where you live by bringing photographic documentary proof of identification and proof of residence with you to the registration meeting.
The following are examples of documents that can be produced, which should be originals and not photocopies:-
- Current Passport (full)
- Full UK Photo card Driving Licence
Proof of Address
- Utility bill – gas / electric / water / cable TV or landline phone bill
- Tenancy agreement
- Council tax bill
- Council rent book
- Household or motor insurance certificate
- HM Revenue & Customs documentation (e.g. PAYE coding notice/notification of working family or children’s or disabled persons tax credit/HMRC tax calculation)
- Bank statement
11:30pm to 1:30pm (Monday to Friday)
If you are staying within the Practice boundary for a short period of time and need to be seen by a Doctor you can register as a temporary patient for a maximum of 3 months. Please inform reception of this and they will ask you to complete the necessary forms.
